CENTER FOR ORTHOPAEDICS PRIVACY PROTECTION POLICY FOR PATIENTS
In compliance with the law and CFO’s commitment to its patients, it is the policy of CFO to protect our patients' privacy.
CFO will:
- Protect the confidentiality of Social Security numbers and nonpublic medical information;
- Prohibit unlawful disclosure of Social Security numbers and nonpublic medical information; and
- Limit access to Social Security numbers and nonpublic medical information.
CFO will not intentionally disclose any individually identifiable health information about our patients or former patients, except as permitted or required by law, or as authorized in writing by our patients. Although complete security cannot be guaranteed, we have implemented procedures to minimize the risk of access or disclosure of individually identifiable health information. (Individually identifiable health information includes, but is not limited to, names, addresses, date of birth, physical or mental health or conditions, medical records including diagnostic tests, lab results and similar medical record documentation, health insurance identification numbers, Social Security numbers, driver’s license numbers, state identification cards, alien registration numbers, account numbers, credit or debit cards.)
CFO makes reasonable efforts to restrict access to individually identifiable health information to those employees who need to know that information in order to provide services to our patients. We make reasonable efforts to train our employees to protect the privacy of our patients. We maintain physical, electronic, and procedural safeguards that comply with federal and state law to guard our patients’ medical information.
CFO will destroy, erase or make unreadable individually identifiable health information prior to disposal. CFO may contract with a business that disposes of confidential patient information. Patient information is only destroyed after being retained the required number of years after treatment ends.
CFO will take disciplinary measures for noncompliance with the policy. All complaints will be investigated and CFO will take appropriate action.
