The billing team at CFO is committed to assisting you through the financial aspects of your treatment as smoothly as possible. We are a participating provider for most insurance plans and networks in the greater New Haven area; please refer to the list below.

Insurance co-payments and/or deductibles are expected at the time of service. Click here to make a secure payment now. We are unable to waive co-payments due to contractual terms with our carriers. Thank you for your understanding.

If a referral is required for your insurance plan, it is your responsibility to provide us with that referral prior to being seen by one of our physicians.

For accurate billing, you must provide your most current insurance information, please notify us immediately if there are any changes to your plan.

A copy of your insurance card will be made at your appointment.
