Some key points you should know are:

(1) Insurance co-payments and/or deductibles are expected at the time of service.  To make a secure payment now click here.  (We are unable to waive co-payments due to contractual terms with the carriers.)

(2) If your insurance plan requires a referral, it is your responsibility to provide us with that referral prior to being seen by one of our physicians.

(3) For accurate billing you must provide your most current insurance information. Please notify us immediately if there are any changes to your plan.

(4) A copy of your insurance card will be made at your appointment.
